Use this form to generate a budget summary report for one or more filing entities or beneficiary schools/libraries.
Enter a Consultant # to view budget information for all entities served by the Consultant.
Select a State to view budget information for all entities in that state.
Enter one or more Filing Entity #s or Beneficiary #s to view budget information related to those entities.
Filing entity reports include information for all schools/libraries included in the filing entity's/entities' application(s).
Beneficiary reports include spending information from all filing entities filing for the beneficiary/beneficiaries.
Enter a file name and type if you want to export the data from the report.

Note: This function retrieves data only for Funding Year 2015. The Line Item Detail report covers all years 2015 and later.
Use this form to export Item 21 data for a filing entity or a beneficiary school or library.
Filing entity exports/reports include information for all schools/libraries included in the filing entity's application.
Beneficiary exports/reports include information from all applications that include the beneficiary.

Use this form to generate a PIA Worksheet for a Filing Entity or Beneficiary.
Enter the entity's ID # to view available budget amounts and funding request line items for all Beneficiaries associated with that Filing Entity, or all requests involving that Beneficiary.
If the requested Filing Entity is a consortium, information for constituent recipients will be shown.

Use this form to generate a Detailed Participant Report for selected FRNs.

Enter a Filing Entity number to see all FRNs filed by that entity.
Enter a Beneficiary number to see all FRNs filed on behalf of that entity.

If the requested Filing Entity is a consortium, information for constituent recipients will be shown.

Use this form to generate a Detailed Disbursement Report based on a Filing Entity, Service Provider, Consultant, and/or Funding Request Number.

Enter a Filing Entity number to see all disbursements made for that entity.
Enter a Service Provider ID number to see all disbursements made for Funding Requests that list that Service Provider.
Enter a Consultant registration number to see all disbursements made for Funding Requests that list that Consultant.
Enter an FRN to see all disbursements made for that Funding Request.
Note: Entering multiple parameters will limit results to disbursements that meet ALL requested criteria.

If desired, select a start and/or end date to limit results.
Choose a format (HTML or PDF) for report output. (HTML format includes option to export to Excel.)

NOTE: QueryBob does not receive detailed disbursement data for all years from USAC. However, we do maintain a record of changes in the Total Authorized Disbursement reported for each FRN. This causes our disbursement date to be a day or two later than the actual disbursement date. In addition, multiple disbursements made on the same day will be listed as one combined disbursement on this report.
